Jardine Lloyd Thompson Pty Ltd have arranged a Personal Accident Insurance Program specifically tailored to meet the needs of Indoor Sports New South Wales and its participating member centres.

A dowloadable claim form is now available from this website. Please see your centre management for further information relating to any claims resulting from an injury sustained whilst playing indoor sports at an ISNSW member centre that is participating in this scheme.

The centre staff will complete an injury report form and fax it to Indoor Sports NSW prior to you submitting the claim form to us. Please remember to have your claim form signed by the centre management before you post it into Indoor Sports NSW.
